Understanding the cost of psychiatric care should not add unnecessary stress to the process of seeking support. CCW Behavioral Health Services offers insurance billing and self-pay options for adults and older adults. Before we confirm your appointment, our team will help you gather the information needed to verify your insurance benefits or use our published self-pay rates.

Using Health Insurance

We help you verify your benefits before care begins

If you plan to use insurance, provide your current insurance information when requesting an appointment. By selecting insurance as your payment method, you authorize CCW Behavioral Health Services to check your eligibility with your insurance carrier before confirming your appointment.

Insurance coverage varies according to the individual plan.

Being eligible for benefits does not necessarily mean that every service will be covered in full.

Depending on your plan, you may be responsible for:

  • A copayment

  • A deductible

  • Coinsurance

  • Services your insurance plan does not cover

  • Charges resulting from inaccurate or outdated insurance information

We recommend contacting the member services number on the back of your insurance card to confirm your mental or behavioral health benefits.

Questions to ask your insurance company

When speaking with your insurer, consider asking:

  • Is CCW Behavioral Health Services, Lekesha McPhail, PMHNP-BC in network with my plan?

  • Does my plan cover outpatient psychiatric evaluations?

  • Does my plan cover psychiatric medication-management visits?

  • Do I have a deductible that must be met first?

  • What are my copayment or coinsurance responsibilities?

  • Is prior authorization or a referral required?

  • Are telehealth psychiatric appointments covered under my plan?

Important insurance notice

Insurance eligibility verification is not a guarantee of payment. The final benefit determination is made by your insurance company after a claim has been processed.

Patients remain responsible for deductibles, copayments, coinsurance, and services that are not covered by their insurance plan.

Deductible Plan Payment Policy

Patients with insurance plans that have an unmet deductible are required to pay $50 at each visit, prior to receiving services. This payment will be applied toward the patient's financial responsibility for the visit. Any remaining balance, as determined by your insurance company's Explanation of Benefits (EOB), will be billed in accordance with our financial policy.
